Louisiana law requires immunization against measles, mumps, rubella, and tetanus-diphtheria. LSU also requires a completed tuberculosis questionnaire for all first-time LSU students. If you do not wish to complete the immunization requirement for any reason, you may complete the Immunization Exemption Form.
Louisiana Residents: LSU automatically receives immunization information for in-state students. In addition to having up-to-date immunization records, you must also submit the TB questionnaire to complete the requirements. You may complete this form, as well as check your immunization compliance status via the Patient Portal.
Use your myLSU credentials to log in to the Patient Portal.
For more information, contact the Immunization Desk in the Student Health Center, 225-578-0593.
PSA STEM students who do not have one of the following must complete the ALEKS exam by May 31 to be placed in the proper Math course this summer.
- ALEKS Math Placement score
- Official math dual enrollment transcript*
- Official ACT Math score of 21+ or SAT Math score of 530+*
To avoid delays in scheduling, please complete ALEKS or submit official test scores as soon as possible. Be sure to log in with your myLSU credentials.
If you took ACT or SAT, your score reports must be submitted via email to admissions@lsu.edu. These can be screenshots of your score report from your ACT or College Board portal. Ensure your full name, test date, and scores are all visible. We recommend using a screenshot from a desktop, laptop, or tablet.
Super scores are calculated by LSU. We need reports from each date you tested. Sending a report of the Super Score or just the highest subscores IS NOT acceptable.
Dual Enrollment transcripts must be sent to LSU by the college/university that administered the credit. Students should visit that school's Registrar's Office web page to request the transcript be sent to Louisiana State University UNDERGRADUATE ADMISSIONS.
Credit will not be awarded from transcripts sent by the individual student.
