No one wants to think about the possibility of death, but it’s important that you ensure your family’s financial security in the event that something happens. Term Life insurance offers financial security for you and your family. Term Life Insurance is offered to eligible employees through two different vendors, UnitedHealthcare and Prudential.

UnitedHealthCare (UHC)

LSU partners with UnitedHealthcare to provide you and your family with Term Life coverage at affordable rates. This coverage provides affordable financial security for your loved ones, especially when your family depends on your income. This coverage is offered to eligible employees, spouses, and child(ren). Employees and any dependents cannot be double covered. This benefit is 100% employee paid.

This plan provides guaranteed issue of coverage up to the lessor of 3 times your annual salary or $350,000, if you enroll within 30 days of your benefits eligibility. If you do not enroll as a New Hire, you can enroll anytime with Evidence of Insurability. Premiums are age-based and will adjust according to age. The coverage amount must be in increments of $10,000. For a listing of rates, please see the Premium Rate Sheet.

Once elected, you can designate your beneficiary in Workday.

Spouse coverage is available on a guaranteed issue basis for up to 50% of the amount of employee coverage, with a maximum of $100,000, if you enroll them within 30 days of your benefits eligibility or within 30 days from your date of marriage. If you do not enroll them within 30 days of their benefits eligibility, they can enroll anytime with Evidence of Insurability. Premiums are age-based and will adjust according to age. Beneficiary designation for spouse coverage is automatically listed as the employee.

You may elect child(ren) Life Insurance coverage in the amount(s) of $5,000, $10,000, $15,000 or $20,000 for each eligible dependent child – no medical information required. Dependent Child(ren) are covered from 14 days to 26 years of age. Premiums are based on the amount of coverage and will not adjust based on age. Beneficiary designation for child(ren) coverage is automatically listed as the employee.


Prudential

OGB offers two fully-insured life insurance plans for employees and retirees through The Prudential Insurance Company of America (Prudential). LSU pays half of the life insurance premium for covered employees and retirees.

Coverage is guaranteed if you enroll within 30 days of your benefits eligibility. If you do not enroll as a New Hire, you and your spouse can enroll anytime with Evidence of Insurability. Dependent child(ren) do not require Evidence of Insurability. For a full listing of rates, please see the Official Schedule of Monthly Premium Rates.

Basic Plan
The Basic Plan offers a flat $5,000 of coverage for employees/retirees. This amount cannot be adjusted. Spouses and child(ren) can also be covered at varying levels, listed below.

Basic Plus Supplemental Plan

The Basic + Supplemental Plan offers coverage of 1.5 times your salary, up to $50,000. This amount will adjust based on your salary. Spouses and child(ren) can also be covered at varying levels, listed below.

Accidental Death & Dismemberment (AD&D) Insurance

Basic and Basic Plus Supplemental Plans

  1. Full-Time Employees
  2. Eligible Retirees

If retired, coverage for AD&D automatically terminates on January 1 following the covered person’s 70th birthday. If the plan member is still actively employed at age 70, coverage terminates at midnight on the last day of the month in which retirement occurs.
