General Information

Eligibility criteria:

  • Enrolled full-time as a graduate student

  • Appointed as a Graduate Assistant in Workday at 50% effort

NOTE: Graduate Assistants are automatically enrolled in the health insurance each semester (fall, summer, spring). If insurance is not wanted, GAs have to waive each semester, even if they waived the previous semester. 

LSU will cover 75% of the total premium cost. 

Please refer to the table found in the Student Health Insurance Initiation Plan Memo for cost information. 

The full student health insurance premium and the 75% credit for LSU’s share will be programmatically added to the account of each GA entered in Workday.

GAs in the Spring will be automatically enrolled in and assessed the Spring/Summer premium. GAs that are graduating in the Spring must contact Bursar Operations at bursar@lsu.edu by the 14th class day to change to a Spring only premium.

The student health insurance plan is non-refundable. 

The student insurance plan cannot be prorated or end early. 

At an additional cost. You WILL NOT receive the 75% subsidy for spouses and dependents. Payment will be handled directly through Gallagher.

  1. Go to www.gallagherstudent.com/lsu-batonrouge.
  2. Log in by following the instructions on the website.
  3. Click on the green ‘Enroll’ button under the ‘Plan Summary’ tile.
  4. Follow the instructions to complete the form to enter and enroll your “Dependent Spouse/Partner” and/or “Dependent Children”.
  5. You are required to upload supporting documentation, e.g. marriage or birth certificate.
  6. You will be prompted to submit payment.
  7. Save Confirmation email will be sent. 

All Fall 2023 GAs should be appointed in Workday before July 12, 2023. The business process in Workday must be completed for the health insurance to be assessed to the student.

New GAs will be appointed as soon as they have all the required documentation. This may not be until a few days before the semester begins. 

 

  1. Go to www.gallagherstudent.com/lsu-batonrouge.

  2. Log in by following the instructions on the website.

  3. Click the yellow ‘Waive’ button under the ‘Plan Summary’ tile.

  4. Follow the instructions to complete the form.

  5. You will need your health insurance ID card.

  6. Confirmation email will be sent.

Dental and Vision Insurance Plans are available to all students to purchase on a voluntary basis at an additional cost. The dental and/or vision plan is available through Humana and WILL NOT receive the 75% subsidy.

NOTE:  Students newly eligible for coverage starting in the Spring semester may enroll between 12/1/2023 and 1/31/2024 for 1/1/2024 coverage effective date.  Please contact your local Member Advocate at (225) 906-1280 with any questions concerning your new benefits.

 

  1. Go to www.uhcsr.com and click on ‘Login to MyAccount’.

  2. Click ‘Create an Account’ if you haven’t created an account or ‘Login’ if you have one.

  3. Complete the registration form using your name, date of birth and student 89 number.

  4. Once logged in, you will have access to ID cards (online or by mail), Claim information, Explanation of Benefits (EOBs)and other plan-related information.

 

Yes, the student must use the services at the LSU Health Center first, where outpatient treatment will be administered or referral issued.  Expenses incurred for medical treatment rendered outside of the Student Health Center for which no prior approval or referral is obtained are excluded from coverage.  Only one referral is required for each Injury or Sickness per Policy Year. Referrals are submitted electronically through GSH’s website.

  1. Medical Emergency.  The student must return to SHC for necessary follow-up care.

  2. When the Student Health Center is closed.

  3. When service is rendered at another facility during break or vacation periods.

  4. Medical care received when the student is more than 25 miles from campus.

  5. Medical care obtained when a student is no longer able to use the SHC due to a change in student status.

  6. Maternity, obstetrical and gynecological care.
