COVID-19 Vaccine Preregistration Survey

Dear LSU Community,
We have been working closely with the Louisiana Department of Health (LDH) and other state agencies to procure COVID-19 vaccines for the LSU community. To ensure that we request the correct amount of vaccine, please complete the preregistration survey as soon as possible by clicking on the link below. Completing this form enters you into a queue for receiving the vaccine at LSU, if and when sufficient quantities are provided by the state. The preregistration is only for vaccines received at LSU and does not mean you cannot receive the vaccine through your private health care provider. LSU will follow the LDH vaccine tier distribution. When a vaccine is available to you through LSU, you will be notified so you can make an appointment to receive the vaccine. At that time, further information will be provided to you with details about when and where the vaccine will be administered. The below preregistration survey will ask for the following information: 
  1. Whether you wish to receive the vaccine at LSU
  2. Whether you have already received the vaccine
  3. Your age and any health conditions that may predispose you to a more severe case of COVID-19. The survey includes only broad categories and does not identify any specific health conditions.
All responses will be kept confidential and the information will be used simply to help us prepare our plan for vaccinating the campus community. We will continue to follow all CDC and state guidelines with the goal of eventually providing a vaccine to everyone who wants one. We will provide more information as it becomes available.
