| Please fill out this form and submit it along with your movie and correct entry fee. Be sure to include a brief description of your submission on the back of this paper or on another sheet. A separate form should be completed for each entry you submit. Clearly label your entry with your name, its title, and its length in minutes. Although this is a “film and video festival,” we currently only accept entries on VHS-NTSC, DVD, and DVD-RW formats only. If your entry is on a format other than one listed and you are unable to have your entry transfered to such, please contact us to see if special arrangements can be made. Please send us a COPY of your movie; it will only be returned upon request. All entries meeting deadline and fee requirements will be viewed by a selection committee, however we can not guarantee that every entry will be shown at the festival.
