Verification Forms

Verification is a review process mandated by the Federal Government, to check the accuracy of information reported on the Free Application For Student Aid (FAFSA) submitted by students and their families. If your application is selected for verification, you will receive notification from our office . Please do not submit any documents unless we request that you do so.

Please refer to your PAWS account to determine if any of the following forms are needed to complete your application for federal aid. If any of these forms are required, you may access them below.
