Immunization Requirements
All students enrolling for the first time at LSU or after an absence of one semester or more must furnish proof of immunization for measles, mumps, rubella, tetanus, diphtheria, and proof of tuberculosis screening prior to enrollment.
To download the immunization compliance form or to obtain more information, visit the Student Health Center’s immunization requirement page. Health forms can be faxed to the Student Health Center at 225-578-5282.
